Camping Checklist This is a new form to our site. It is a general checklist for camping trips. It contains most of the items each Scout and adult should carry for all Troop camping trips. Use it as the basis for packing and add items specific for each trip depending on the activity and weather.

Patrol Forms and Information

Patrol Equipment Checklist Use this checklist to ensure that your Patrol has all your Patrol equipment in good condition before and after camping trips.
Patrol Roster The Patrol Roster is used to list telephone numbers and other information about each member in your Patrol.
Patrol Call Log The Patrol Call Log has a place for you to add each Patrol member and to record information you need to tell each member. It also has a place to take notes on who is coming to the meeting and the camping trip.
Patrol PLC Notes The Patrol PLC Notes page is designed for you to take notes during the PLC. The Senior Patrol Leader can use this form to fill out information before the PLC and give each Patrol Leader a partially filled out set of notes.
Patrol Weekend Duty Roster You record on the Weekend Duty Roster the jobs assigned to each Scout for a camping trip. The Roster must be posted in your Patrol site.
Patrol Menu Planner The Menu Planner is used to plan your menu for a camping trip. It must be filled out, turned in to the appropriate adult, and approved in advance of each camping trip.
Patrol GrubMasters List The GrubMaster prepares this shopping list based on the Patrol Menu and the number of Scouts going camping.
Patrol Leader’s Notebook This is the complete Patrol Leader’s Notebook. It includes the table of contents and all the supporting leadership information.

Troop Forms and Information

Troop 565 Leadership Guide The Leadership Guide defines the major requirements and responsibilities of the Senior Patrol Leader, Assistant Senior Patrol Leaders, Senior Patrol; Members, Patrol Leaders, and Assistant Patrol Leaders.
Troop Meeting Planning Form This planning form should be prepared for each Troop Meeting and presented to the PLC prior to the meeting. Revised plans should be posted on the bulletin board prior to the start of the meeting. Submit completed forms to the webmaster for posting on the Troop website prior to the meeting.
Camping Trip Planning Form Use this form to plan and schedule camping trip activities. The completed form should presented to the PLC prior to the camping trip. Revised plans should be posted on the bulletin board during the camping trip. Submit completed forms to the webmaster for posting on the Troop website prior to the camping trip.
P.A.S.S. Sheet This document contains the general instructions for P.A.S.S., the Proficiency Award for Scoutcraft Skills. It also contains the more detailed sign-off sheet used to track a Scouts progress towards P.A.S.S.
P.A.S.S. Skills Instruction Manual This document is a skills instruction manual for some of the items tested in PASS. It does not cover the entire text. However, if provides invaluable information for Scouts attempting to pass the tests that it does cover. Must reading for all PASS candidates. (2MB)

Administrative Forms

Check Request This form must be filled out and given to the Troop treasurer along with receipts to be reimbursed for legitimate, approved, expenditures. This form is available at Troop meetings.
Troop Activity Permission Slip Troop 565 Parental Consent and Medical Release must be completed for all Scouts before they can participate in the outdoor program. It should be submitted directly to the Scoutmaster.
Medical Form for Scouts Class 1 and 2 Personal Health and Medical Record. Each Scout must have a physical once every 36 months and have this form completed by the physician. In addition, each year the parents must update the first page of this form and submitted directly to the Scoutmaster.
Medical Form for Adults Class 3 Personal Health and Medical Record. This form must be completed by all adults participating in major activities, e.g., summer camp and rock climbing, and Scouts participating in high adventure programs, e.g., Philmont and Seabase. Submit completed forms directly to the Scoutmaster.
Troop 565 Medication Form This form must be completed by a parent or guardian for each prescription and non prescription medicine that a Scout is required to take on a regular basis. The form should be put in the same plastic bag with the medication.
Climbing Release This form must be read, signed, and returned to the Scoutmaster prior to any Scout or Adult participating in Troop sponsored rock climbing or rappelling. This form must be completed annually.
Driver List This form can be used to track the drivers for trips. It contains a place for the names of the drivers and the amount of passengers going and returning with each driver.
565 Vehicle Insurance Information This form needs to be filled out once for each car owner / driver who will transport the scouts to their scouting event.
